Ben Gustafson; RMT is a provider of massage therapy services for American Speciality Health Clients and will honor verified policies or plans under this system.  This includes the "Affinity Plan."  When calling for an appointment please have the following information from your health plan identification card on hnand.

1. Your name as found on the card.
2. Your birth date.
3. Your health plan identification number.
4. The health plan name.
5. Employer name and employer group number.

This information will assist in the verification process and insure you obtain your member benefits.
